Below you will find a series of new patient information forms and consents. In order to facilitate a timely and efficient clinic, we ask that you either complete these forms prior to your appointment or arrive 15 minutes early to allow ample time to complete them in our office.
At the time of your initial visit, you will also need a form of identification and your insurance card(s). Without a copy of your insurance card(s), we cannot bill your insurer. If you forget your card(s) (or copies), you may be asked to pay for services at the time they are rendered. If the patient is a minor child, a parent or guardian will need to be present. Please note that we do not accept or file for Medicaid. If you have Medicaid as a secondary you will be responsible for that amount not covered by your primary insurance.
Additionally, for your initial visit, we ask that you limit your concerns to one or two problems. This allows the physician ample time to address your problem(s) and ensure that you understand your condition and treatment plan. This is especially important for more complex problems such as psoriasis, hair loss, skin cancer evaluations, atopic dermatitis, and many long-standing dermatoses. We will gladly give you another appointment at a later date to address any other issues.
